When setting template sources note that there are Document Options for the entire document or the current sheet at the top of the Properties Panel.
Example for a size B Fab Drawing:
Create sheet 1, sheet 2 DwsDot
Create a DwfDot using sheet 1 and sheet 2 DwsDot.
Add fab notes and logos to the DwfDot.
Then save the DwfDot as PCBDwf.
Add the PCBDwf to your project.
That's it !
